Records from the Spring and Autumn period connect the surname with descendants of the rulers of Song, Chu, and Lu. During the reign of Emperor Ming of Han, it was changed to Yan to avoid taboo, and some Yan families restored the Zhuang surname during the Wei, Jin, and Southern and Northern Dynasties.

Before the Qin dynasty, the surname was mainly active in Shandong, Hubei, and Hunan; after the Qin and Han, it spread to Hebei, Jiangsu, Zhejiang, Guangdong, and elsewhere. In the late Tang, Zhuang Sen entered Fujian with Wang Shenzhi, and in the early Qing, Zhuang families from Fujian and Guangdong entered Taiwan.
